Add Food: Field label to choose category is missing

A clear and concise description of what the problem is. Ex. I'm always frustrated when [...] After adding Title and Description, a dropdown box is presented. Without any label, its not clear what does it mean and leave a user to fumble around.

Add field label "Choose Category" above the field.

Start at the add-food form described in the issue, after the Title and Description fields, and locate the unlabeled category dropdown. Add the requested “Choose Category” label above that field, then verify that the label is visible and clearly associated with the dropdown.
