Invalid field for entering phone number ,hence phone number not taken

To Reproduce
Steps to reproduce the behavior:

Go to ADD RIDER
ENTER CREDENTIALS
MOVE TO PHONE NUMBER
See error
Expected behavior
Should be a numeric textbox

Reproduce the issue in Chrome by opening ADD RIDER, entering credentials, and reaching the phone-number field. Inspect the form entry point for that field and verify that it accepts a phone number as a numeric textbox without the reported error.
