Description
Copilot app 1.0.84-5 on Windows 11.
After a session runs for some time, every tool invocation causes:
Failed to refresh MCP tools for server github-mcp-server;
the last catalog is stale.
Dynamic header provider failed:
GitHub credential registration is no longer available for this session.
The failure appears even for local PowerShell commands and local file reads.
The underlying tool sometimes completes successfully, but the UI reports
execution failure. Restarting or starting a new chat temporarily resolves it.
It recurs in most longer chats. Uninstalling WorkIQ had no effect.
