Description
Confusing blocking behavior and incorrect guidance from ask question tool related to planning
- pretty sure I was not even in plan mode when I sent the initial request - so confusing that it claimed I was
- edits do sometimes get made while in plan mode from my experience, and if not, it'll usually just create a plan for making those edits. so confusing this was surfaced as a blocker at all
- switching modes in the middle of a session, esp with an active question open like this is not possible
- shift+tab shortcut for switching modes is also outdated for ghcp app - although it may still the case for CLI

Research direction
Start by reproducing the reported behavior in the /chat path using app version 1.0.25 on Windows, checking planning-mode guidance, edits, active questions, and mode switching. Compare the displayed instructions with the actual behavior, including the shift+tab shortcut. Done means the guidance and blocking state accurately reflect the current app behavior and supported mode switching.
