Starting a new session in a repo fails occasionally with with one of the following behaviors:
1. Left nav bar just says "New Session" and the chat just runs indefinitely until you delete the session.
2. The sessions stops running, no error message, but your initial prompt is still there. The worktree seems to be created internally, but never reflected in chat.
3. The worktree gets created, shows in the chat session, but the initial prompt is lost.
Currently, starting a new session in a repo only works ~ 50% of the times.
