Wallet connection issue on request money screen using Fortmatic/Metamask

**Describe the bug**
The wallet shows connected to Gitcoin in the top right and shows the address in the modal box to start.

Once I enter another field such as the person I am requesting from, the address dissappears.

I am then not able to hit the "send" button because the fields are not filled out.

Cannot figure out how to get the wallet to show up again. Copy paste, disconnecting and reconnecting, none work.

Go to gitcoin.co/requestmoney

Use fortmatic wallet and attempt to send a request for money.

**Expected behavior**
The form fields should be easy to fill out and then hit send when wallet is connected.

Connected Metamask afterwards and it seemed to work, but instead ended up with an Infinetly sending and never completing action.
