Jira Integration Problem - Multiple Jira Transitions

### Steps to Reproduce

1. In Jira, create multiple workflow transitions to a single status.
2. In Sentry, open Settings > Integrations > Jira > Configurations > Configure.
3. Open one of the "When Resolved" or "When Unresolved" dropdowns and search for the status.

### Expected Result

When multiple statuses with the same name appear, show `[Status Name] - [Transition Name]`.

### Actual Result

Multiple statuses with the same name appear, it is impossible to distinguish between the different transitions other than by trial and error.
