Startup failed: Failed to create server

I'm trying install a shiny app via a package hosted on bitbucket.

The app builds and installs without reported issue. When I run it, is see "Startup failed with error(s): Failed to create server".

I'm not clear what I'm missing, but it feels like the article on continuous installation may be missing a key step or two.
