Segmentation Result in Cut Outs showed an unexpected sharp triangle

I encountered a problem that appeared to be at the software development level (rather than the neural network level).

When I added guide points to the image during segmentation, everything looked normal, but the segmentation result in Cut Outs showed an unexpected sharp triangle.

The report names no file, notebook cell, or test; start by reproducing segmentation with guide points and comparing the normal preview with the Cut Outs result shown in the screenshots. Done means identifying the cause of the unexpected sharp triangle and confirming that the resulting cutout no longer contains it.
