Document crate organization

Document the dependency graph between our crates as well as the intended scope/separation of concerns of each.

Start by inspecting the repository's crates and their dependency manifests, then read each crate's existing documentation to understand its intended scope. Done means the documentation explains the dependency graph and the intended separation of concerns for every crate.
