Add tests for real use of the embedded and http adapters

We've had a couple of issues related to differences between the Plug test adapter and the real behaviour in Bandit. ATM we have an example app that allows for manual testing for both HTTP and embedded modes, but it's tedious. We need ideally unit tests that reproduce a real-world app or alternatively CI integration tests.
