Add supporting diagrams to the app

## User story
As a student user,
I want to be able to see supporting graphics representing the data I see on screen
So that I can make better, contextual decisions.

## Description
Configure the input spreadsheet to allow embedded images to appear in forms.
On the wound assessment page add a diagram with a patients head and the 12 numbers around a clock.
On the respiratory assessment page add a diagram with the lungs.

## Expected behaviour
User can see diagram of lungs on the respiratory "add new" modal form.
