Server side S3 SDK example

A server side example might be easier to implement for someone who already has an existing code base. The [direct form upload example](https://github.com/dwyl/image-uploads/tree/master/examples/direct-upload) might clash with existing forms because they can't be easily [nested](http://stackoverflow.com/questions/379610/can-you-nest-html-forms).
