Lacking information on SQL Server setup to support SqlChangeMonitor

Use of SqlChangeMonitor requires specific set up in SQL Server, but this SqlChangeMonitor page makes no mention of that dependency and does not point to documentation explaining the necessary steps. I have so far been unable to find such documentation, relying instead on aggregating information from posts of other peoples attempts, successful and otherwise.
