Missing migration steps to migrate from 4.0.x to 4.5+

This document only mentions migrations from WF3 to WF4 and there are no steps for migrationg WF4 to WF4.5.

[What's New in Windows Workflow Foundation in .NET Framework 4.5](https://docs.microsoft.com/en-us/dotnet/framework/windows-workflow-foundation/whats-new-in-wf-in-dotnet) hints on some of the necessary steps, but it's not clear what they are.
