Add/Remove/Update rate limits from config, reloadable

### Is your feature request related to a problem? Please describe the problem.

Applications encountering issues in production will want to add, remove, or tune their rate limits on endpoints without recompiling their application, or even better, without restarting.

### Describe the solution you'd like

Establish a pattern and APIs for defining rate limits in config and associating them with endpoints. Ideally these would reload on change, but avoiding a recompilation is the minimum requirement.
