Create GH pull request template [DBZ-4599]

Let's create a [template for pull requests|https://docs.github.com/en/communities/using-templates-to-encourage-useful-issues-and-pull-requests/creating-a-pull-request-template-for-your-repository] against the different Debezium repos. It should be as light-weight as possible, probably containing not much more than that:

- A link the [contribution guide|https://github.com/debezium/debezium/blob/main/CONTRIBUTE.md]
- Perhaps point out key things to consider (commit message format, formatter must have ran)
- A basic check box list: tests, documentation updated, change log notes provided (if applicable), license checked (after major dependency updates)

Start with CONTRIBUTE.md and the linked GitHub guidance for pull request templates, then identify the Debezium repositories that need the template. Done means each applicable repository has a lightweight pull request template linking the contribution guide and covering the listed checks, guidance, and conditional changelog notes.
