Description
I have `page-break-after: avoid` set for h1-h6. If I generate a link to a heading that would have rendered at the bottom of a page, but now has moved to the top of a new page, then the link target is still pointing to the bottom of the first page. Bookmarks etc also have the incorrect page number.
