Description
I have a custom FHIR registry entry in ~/.fhir/fhir-settings.json, e.g.:
{
"ignoreDefaultPackageServers": true,
"servers": [
{
"url" : "http://localhost:8103/packages",
"type" : "npm-package",
"authenticationType" : "token",
"token" : "njiXXXXXXDMugkidY"
}
]
}
When I clear my ~/.fhir/packages, then launch VS Code, the dependencies are downloaded from the public registry and not the one specified in fhir settings.

Research direction
Start by tracing dependency loading when the VS Code extension launches, including how ~/.fhir/fhir-settings.json is read and how registry entries are selected. Reproduce after clearing ~/.fhir/packages with ignoreDefaultPackageServers enabled; done means dependencies download from the configured local registry rather than the public registry.
