Add deployment documentation, consider adding scripts and/or templates

Leasing depends on underlying storage. (Currently, Azure Blob Storage is the only supported store.) This means that to deploy an application that uses leasing, you must deploy a storage service. We need to document the requirements, including examples of how to ensure how that configuration settings with the relevant details are available to the leasing code.
