As program manager I want to send follow-up surveys to program hosts

For example, if we need the special diets of the program hosts, we may not have asked this when gathering program offers (as we do not need -> cannot ask that info at that stage; part of the offers may be rejected).

The follow-up survey responses should be connected either to the program host (person) or the program item (or both).
