Privileged access document that contains contact information for Clean Slate partners/resources/etc

Related to codeforboston/clean-slate-data#166 and codeforboston/clean-slate-data#165

## Goal
Create a document that contains contact information for Clean Slate's stakeholders/partners/resources. The idea is to have a shareable document and enough trusted people with keys so the information has a lower risk of being lost.

The reason for keeping the information more private is 1) avoid the information leaking out to the public and 2) require formal introduction between a contact, the person wishing to reach out, and someone the contact is familiar.

Start by reading the two related Clean Slate Data issues (#166 and #165) and confirm where the stakeholder information is intended to live. Clarify the approved privacy and access process, including trusted key holders; the work is done when the contact document and its access arrangements are explicitly agreed and documented.
