Change Accept semnătură digitală option from checkbox to Yes/No

In the form details page of the profile, change the Accept semnătură digitală option from a checkbox (as is currently) to a mandatory, Yes/ No question.

- Accept semnătura digitală - mandatory, Da/ Nu answer options (+ brief descriptive text about what this means - contributors can sign online, and if they choose to do so, the form is valid and can be downloaded by the organization through Redirect/ if not, the form will have to be printed, signed and sent to the organization by the contributor).
- If the answer is Nu (to the Accept ...) - add a brief disclaimer that if not accepted, the forms will have to be printed, signed and sent to them by the contributors for them to be valid + cannot be downloaded by them from the organization's profile.
