[AMC/ Beneficiari/ Catagrafie] Implement dynamic field “Denumirea afecțiunii” when selecting “Alt tip de afectiune”

**USER STORY**
As an AMC user, when I select “Alt tip de afectiune” value for "Boala sau categorie de boala" field, I need to be able to have an input field where to note the name of the illness/ medical condition.

**DESCRIPTION**
Implement in Catagrafie/ Stare de sănătate - detalii section a new input field labeled “Denumirea afecțiunii” which will be displayed when selecting “Alt tip de afectiune”.

Start in the Catagrafie / Stare de sănătate - detalii form and inspect the existing “Boala sau categorie de boala” field and its handling of the “Alt tip de afectiune” option. Done means an input labeled “Denumirea afecțiunii” appears only for that selection and records the illness or medical-condition name.
