add versions and changelog to reference architectures

Currently we track the submission and updated date.

During today's public meeting, there was the idea to add versioning to the reference architectures along with a changelog. We can add version metadata at the top with the changelog at the bottom of the document.

Locate the reference architecture documents and inspect how the existing submission and updated dates are represented. Add version metadata near the top and a changelog at the bottom, following the project's current document structure. Done means the reference architectures consistently show both the version information and change history.
