Description
As an author in Joplin, I need a way to communicate questions and issues to the Joplin team.
Design issue: #4913

Acceptance criteria:
- [ ] Insert a link as designed in the left nav to the form
- [ ] From the link, a form pops up in a modal (lightbox)
- [ ] Modal displays as designed with text and drop down widget
- [ ] Modal should be re-sizeable (so that the text field has a bigger entry area)
- [ ] When the user clicks send, the form data and specifics from Joplin are sent in an email
- [ ] See comp for specs about the user and browser that should be sent within the email
- [ ] When the user clicks send, the modal is dismissed.
- [ ] Each entry is logged in a github repo like our Janis feedback
