Expand form container capabilities

As forms have been increasingly well-developed, there are many services that are basically just about submitting the form. For example:

As a content strategist I would like to be able to surface these form containers as top services within topics and in a department (related pages would be fine, too). For some departments, like DACC, this is one of only one of two pages. For some topics, like Public Records, the form container is the only page related to it right now.

I would also like to be able to attach a contact to a form container.
