The service page for reporting diseases was written a long time ago.
- [ ] Use the [service page template](https://docs.google.com/document/d/1ZHkXPnn37d88bTPZzov7hMjOYM9IrvIunm1u3FcrW-4/edit) to make sure we have the content we need.
- [ ] Rewrite the page if necessary. Is the language service-oriented? Is it well-organized? Could it be more concise? Remove anything that is out of date.
