As a young person, I would like this form to be clearly formatted and up to date so that I know my input is taken seriously and my time is valued.

Current form:
- is a downloadable, printable word doc (not digitally fillable)
- is out of date by two years
- has othering gender language, no self ID option
- has many formatting issues
- has non-current program leads on arts programs being assessed (employees have left some/all programs)
- ends with juvenile crime as its last question
**What is the long-term, ideal solution for this content debt?**

Uncertain, but requires thoughtful reinvention to address tone, format, delivery, and usability.
Consider:
- what are the outcomes for this survey
- how could this form better support the CYD program coordinator's role?
- what is the audience and the desired relationship with them
- could a digital form or a more targeted delivery of the survey work well?
- can this be written in a way that makes it usable regardless of staff changes?
