Incremental mode in affiliate command

When doing affiliations, just work by default with identities who don't have an affiliation already.

Start by locating the affiliate command and its current identity-selection logic. Confirm how affiliations are applied, then make the default behavior skip identities that already have an affiliation; verify the command still handles identities without one as expected.
