Bootstrap GitHub workflow, templates, CI skeleton, and main protection

Outcome

The repository supports small issue branches, evidence-bearing pull requests, required checks, and protected integration.

Scope

• Add lightweight issue and pull-request templates.
• Add the decision-record template used only for contract, invariant, or security changes.
• Add CI skeletons for backend formatting/tests, schema/replay checks, secret scanning, and frontend builds.
• Configure main-branch protection and required checks.

Acceptance

• A new issue can state scope, acceptance checks, dependencies, and review needs.
• A pull request prompts for tests/evidence and security/schema impact.
• CI runs on a sample pull request and reports each configured check.
• main rejects an unreviewed or failing change according to repository permissions.
• No project board is required unless separately chosen by the product owner.

Dependencies

• None; can run in parallel with external account setup.

Review

Product-owner verifies branch protection; implementation may be reviewed by any independent model.
