Related to issue: #5098
## Modal to collect end-user contact and report error (support flows)
Key elements:
- Update to error messages to have a button to open the modal
- Modal with an input field to gather the end-users contact information
- gc-ds Notifier component to let users know that the error has been logged
- Once submitted, 2 emails will be sent to the end-user and to the contact email on the form
