# Issue / opportunity
When adding a form element, there are inconsistencies in how the elements are inserted in the form edit view.
# Examples / screenshots
- Radio buttons (no icon) vs. checkboxes (icon)
- Full name (no type) vs. Individual names (short answer)
# Expected behaviour
1. Remove the icon svg from "type" example text of "checkboxes" when the form element is added for consistency
2. Add short answer "type" example text to "full name" when the form element is added for consistency
