Link to the exact VS-Code extension

1. Searching for `Continue - open-source AI code assistant` still presents multiple results (and I think this is supposed to be `Continue - open-source AI code agent`. I think there should be a link to the correct extension so users can verify that they're downloading the correct one, which I think is: https://marketplace.visualstudio.com/items?itemName=Continue.continue
2. Not an issue but perhaps at some point we can highlight setting [context providers](https://docs.continue.dev/customize/deep-dives/custom-providers#%40repository-map) (of course without replicating whatever is in the upstream docs). In my initial testing, these really help to focus what the inference snap is used for here including using a repository/folder as the context (with imperfect results so far, but it's a start).
