Show organization as "disabled public donations" in "add to salt" and "create fundraiser" dialog

In the dialog: https://www.bountysource.com/teams/new?salt=true

it shows team I am member of. However: if the team specifically turned off donations:


and you select it and go to "fundraiser", it says:


which redirects again back to salt, resulting in an endless loop.

I think the best solution would be to:
- Remove the link under "fundraisers" back to salt if the setting is setup and add a notice that "you disabled public donations"
- Add a icon, visual helper with rollover in the list of teams that notes that the project has public donations disabled.

Start at the add-to-salt dialog at https://www.bountysource.com/teams/new?salt=true and reproduce the redirect loop by selecting a team with public donations disabled, then follow the fundraiser flow. Done means disabled-donation teams are clearly identified and the fundraiser flow no longer redirects endlessly to Salt.
