Feature Request: Custom tick box

Hi Nigel

Would it be possible to add a tick box on user form to display a line of text in the submissions?
With the following on the configuration page...
Tick box for if its displayed on the form, Editable text label for tick box the text is displayed on the submission?
Just needs to be visible on submissions.

The reason for this is our region provides zoom bookings for meetings maybe other regions may have uses for this too :)
