Authenticator items synced from vault bug

### Steps To Reproduce

I’ve noticed that some of the entries synced from my vault have issues.

- Missing titles & Usernames just the URL

- Incomplete, missing Usernames, partial Usernames,%2B showing in Usernames when it should be a +

They show ok in the verification codes in Bitwarden.

The majority of entries are correct.

Just checked on the iPad as the screen shows more of the line and the entry with a %2B also has a %40 and %2E where it should read:-

firstname.lastname+vendor@gmail.com

Entries should show as per the entries in Bitwarden

Incorrect looking text fields in Authenticator
