Insert key not working

This is a holdover from https://github.com/bitovi/syn/issues/18

The Ctrl / Alt / Meta was a different syndrome than insert, and I wanted to close those issues.

Insert is another beast entirely. For me, it will actually hang Qunit. Timeouts cease to function, and the test will never finish.

Start by reproducing the Insert-key case in QUnit and tracing the library's keyboard-event path; the issue names no file or test entry point. Done means Insert no longer hangs QUnit and its timeout completes.
