Update documentation related to ignore, to clearly explain that we don't have an AST

Description
We caused confusion for @Trott with the language `next` for `/* c8 ignore next */`, because he assumed (understandably so) that this would ignore the next logical element in the AST.
We should make the docs explain our reasoning better.
