As a staff person I want to prepare and submit a recommendation package to the DM for decision on a plan so that my recommendation is stored with the digital record (and I don't have to make a paper copy of the plan)

**REQUIREMENTS**
- [ ] initiate a decision package
- [ ] provide background (could be a few paragraphs of text) **(required)**
- [ ] enter "Consultation Summary" (could be a few paragraphs of text)
- [ ] enter hyperlink text for "Consultation Record URL" **(optional)**
- [ ] link opens in new tab
- [ ] attach consultation record (ex. PDF document) **(optional)**
- [ ] enter "Stakeholder Summary" (could be a few paragraphs of text) **(optional)**
- [ ] enter hyperlink text for "Stakeholder record URL"
- [ ] link opens in new tab
- [ ] attach stakeholder review record (ex. PDF document)
- [ ] select "Range staff recommendation" with options "Approve", "Approve with Conditions", "Not Approved - Further Work Required", "Not Approved"
- [ ] if "approve with conditions" is selected THEN the staff person can one or more conditions
- [ ] each condition is entered separately (so that one but not all could be selected by decision maker)
- [ ] enter text in "proposed conditions"
- [ ] if "Not approved - further work required" is selected THEN the staff must describe the further work required in a field labelled "Description of recommended further work required"
- [ ] submit decision package to decision maker (as identified for district)
