Add a new entry to a fuel supplier's compliance log

Description:

As an IDIR user, I want to be able to add a new entry to a fuel supplier's compliance log.  I would like the log to have the following:

- Can add individual entries relating to a fuel supplier organization
- Track/show: entry, user who made the entry, date/time
- Free text, preferably HTML (for body)
- Date field: calendar selection; current date default
- Record #, ID #
- Search/filter functionality
- Categories
- Have pre-defined categories for entries?
- Examples: Compliance Reporting, Fuel Supplier Correspondence
- Access: IDIR users only
- Log should be for compliance issues only, not other items (e.g., P3A milestone submissions)

Purpose:

So as an IDIR user can maintain an up-to-date record of compliance behavior.
