**Describe the Bug**
Report 209 in the test environment appears to be experiencing multiple issues related to reassessments. A reassessment was previously set up on the report but now appears to have disappeared. The Reassessment button was initially unavailable, and subsequent reassessment actions appear to be creating or referencing a different report ID (350) instead of the original Report 209. The report is now also returning a server error.
**Expected Behaviour**
- Report 209 should retain its reassessment history and associated records.
- Reassessment actions should remain linked to Report 209.
- Any reassessment created from Report 209 should follow expected business rules and maintain proper report relationships.
