LCFS - BCeID CI application Step 5

**Description:**
Create Step 5 of the CI application process for BCeID display during government review and decision by IDIR users.

**Purpose and benefit to user:**
Allows BCeID users to review their applications, communicate with government staff, and add additional documents if requested by government.

**Acceptance Criteria:**
- Application is displayed in fully locked format.
- BCeID users can add comments.
- Supporting documents can be added if government staff request and enable the upload utility.

- [ ] Enable conditional document upload

**Notes:**
- Steps 1–4 are hidden after submission.
