Description
**Describe the bug in current situation**
A clear and concise description of what the bug is.
A Public Body Applicant Consent extension was requested from, and denied by, the applicant. This is reflected accurately in the 'Extension Details' section in the Mod site (and in Axis). However, I noticed that in the Request History Comments it says "The OIPC has denied a 30 day extension", not the applicant.

Research direction
Start by reproducing the denied Public Body Applicant Consent extension and compare the Extension Details section with the Request History Comments in the Mod site and Axis. Trace where the history comment is generated; done means the comment identifies the applicant as the denier and existing extension behavior remains accurate.
