Make "Name Request" page title clickable to take user back to start screen

Description
It's common for users to be able to click the main title to be able to "return to the main/home page" as we do with the BC Reg logo and the BC Registries and Online Services title in the header.
Would be good to let users click the "Name Request" header to return to the main screen (would do the same thing as the "Start Over" button on the search screens)
