Description
On the live Name Request site, in error, we are including corporate designation dropdowns for CCC name requests.
We need to remove the designation dropdowns and have the name fields (1,2,3) go all the way across like we do with Coops. CCC's should not be selecting a corporate designation.
To see this bug:
1. Search to Register or Incorporate a BC Community contribution company "Tracey's Donuts"
2. On the Submit a NR screen, we are required showing corporate designation dropdowns that shouldn't be there.
