GOVM- Phone number field needed

**Describe the bug in current situation**
As a GOVM admin, when I am creating my account, my phone number is not captured in the workflow.

**Pre Conditions: which Env, any pre-requesites or assumptions to execute steps?**
- Using IDIR create a GOVM Account
**Steps to Reproduce**
Steps to reproduce the behavior:
1. Go to the email link and log in using Test IDIR
2. Accept TOS
3. Go through the account creation process
4. See error on the 'Contact Information' screen

**Actual/ observed behavior/ results**
Phone number field is not captured during GOVM account creation workflow

**Expected behavior**
An editable field, Phone Number should be visible during the account creation process
