Location in Business Registry for Administrator Request Form

* As a Business Registry Account Holder
* I want the ability to request BCOL Admin add an administrator, upgrade a team member to an administrator, or remove an administrator when our existing administrator on file is unable to do so
* so that I can maintain my account

**Acceptance Criteria**

**Scenario 1: Business Registry team requires an administrator is added, team member is upgraded to administrator, or administrator is removed **
* GIVEN the existing administrator on an account is unable to make necessary changes to the team members
* WHEN I navigate to the Business Registry
* THEN I want to be able to locate a form which allows me to request that BCOL Admin makes changes to my team on my behalf

**This ticket has been created to establish the location of the form. The form will need to be posted somewhere on the Business Registry. Loren has suggested it may land within the Team Members UI. A client may also be unable to log in when making a request (e.g. account is suspended) so review is needed to determine if there is another location as well for these users. Users who cannot log in could also obtain the form directly from Registries Staff.
