Use Relationhips masthead for Name Request (Logged in state) - UI DESIGN

Used the Relationships masthead component for Name Requests - this will allow the user to log in and have their Name Requests affiliated to their account.
Includes the update to the page title (remove ampersand)
The user can log in from any name request page/state. After logging using their choose log in credentials they are returned to the Name Request site on the same page they left. The page is in the same state as when they left (other than the logged in state). This includes auto-approval pages and applicant pages.

The implementation will need to assess whether or not the Relationships component (Responsive) can be included in the current Name Request page layout (fixed width?) or if the Name Request page layout need to change.
