Outline/wireframe for Form 47 Correction (All filings) - UI DESIGN

For staff corrections, clients will submit a paper version of Form 47 - Register Correction. Users will complete the form, submit it to staff and staff will file the correction.

**Name Request**

Staff will require Name Request information to change the name of the company using an NR
NR Number + Name Request Applicant email, or Name Request Applicant phone number

**Supporting Documents**

Clients will be asked to provide supporting documents before staff will make the change.

I am in receipt of your request to file a Form 47 Application to Correction for the above noted-company. I require supporting documentation before I can proceed with the filing.

Appropriate supporting documentation proving the error was typographical only would include any one of the following listed below:

1. Incorporation Agreement and Articles of Incorporation, signed and dated on or before date of incorporation/registration/resolution.
2. Paper filing of said Form, signed and dated on or prior to date of incorporation/registration/resolution.
3. Special Resolution, signed and dated on or prior to filing.
